simple solution. This excel document includes two spreadsheets
that can help you determine sample size when surveying stores
in your community for illegal sales to minors. (See Document

Youth Interview
Form
Once you have a list of teens who want to participate in your
Youth Purchase Survey (YPS), it is usually best to pre-screen
them before inviting them to a training session. This telephone
interview form provides typical telephone screening questions.
Youth Profile
Form
This form can be used during the youth recruit training session
to gather information about the youth. (See Document
the Problem: Store Marketing section of the manual (pg. 8)
for more information about youth training.)

Permission Forms:
All teens who participate must have written permission from
a parent or guardian. In addition, it is best if the teens themselves
sign a permission form to demonstrate that they, too, understand
