Deciding how many stores to survey is a difficult issue with no simple solution. This excel document includes two spreadsheets that can help you determine sample size when surveying stores in your community for illegal sales to minors. (See Document the Problem: Store Marketing section of the manual (pg. 2) for instructions on how to use.)

Youth Interview Form
Once you have a list of teens who want to participate in your Youth Purchase Survey (YPS), it is usually best to pre-screen them before inviting them to a training session. This telephone interview form provides typical telephone screening questions.

Youth Profile Form
This form can be used during the youth recruit training session to gather information about the youth. (See Document the Problem: Store Marketing section of the manual (pg. 8) for more information about youth training.)

Permission Forms:
All teens who participate must have written permission from a parent or guardian. In addition, it is best if the teens themselves sign a permission form to demonstrate that they, too, understand the nature of their participation. The following are sample consent forms, provided in both English and Spanish.

Survey Protocols:
  Standard Protocols - This protocol is used in the statewide survey. It is also the protocol most often reported in the literature.

Lying Variation Protocols - A variation on the Standard Protocol; in this variation, the youth lie about his/her age.

Flash ID Protocol
- The Flash ID Protocol is similar to the protocol the Food and Drug Branch of the California Department of Health Services uses in their enforcement activities.

Youth Purchase Survey (YPS)
Use this survey instrument to collect data on sale outcome, price, type of cigarette (or tobacco) requested, and even store advertising, following the youth purchase attempt.

Stanford Enforcement Survey Database  - word
Local tobacco control ordinances that your community may have passed are contained in the municipal code (this is the city’s or county’s book of laws). This document contains the types of ordinances that your community may have passed as of September 2000. Survey conducted December 2000 through February 2001.
